Use this question when you want respondents to upload an image to the survey as a response.

To add an Image Response question, click on the icon or drag and drop from the menu on the Survey Editor's left side.
Make sure to provide the respondent with clear directions on what you want from them. In addition to your question, you can add an image or additional instructions for context. Respondents can use their phone or webcam to snap a photo or upload one of several types of image files as their response.
As you set up this question, you can choose a few other options, such as making the question optional (meaning your respondents can skip the question entirely) or requiring an upload rather than a captured photo.
This checkbox labeled "QR" is for receiving responses to in-home product tests – respondents can directly scan a QR code from a product package or enter the product code manually, even using a specific format if you need them to.
To review image responses, scroll to the Image Response question in your survey and click the phone icon. This will open a new page with only image responses. Here you can browse through the images, which include each respondent's demographic traits, or you can download all image responses to a zip file.
Image responses are also included with open-ended responses, if you click the icon at the top of the results to open the open-ended text responses any image responses will be
included as well.
